"Creatine damages your kidneys" is the most persistent claim about the most studied supplement in sports nutrition. It has a specific origin, and understanding it resolves most of the confusion.

Before the detail: this is general information, not medical advice. If you have kidney disease, reduced kidney function, or take medication affecting the kidneys, talk to your doctor before taking creatine. The safety data below describes healthy adults, and that distinction matters.

Where the concern comes from

Kidney function is commonly estimated using serum creatinine — a waste product filtered by the kidneys. When kidneys aren't filtering well, creatinine accumulates in blood. So elevated serum creatinine is a standard red flag.

Here's the problem. Creatinine is the breakdown product of creatine. Roughly 1.5–2% of your body's creatine pool converts to creatinine every day and is excreted.

Increase the size of the creatine pool through supplementation, and you increase daily creatinine production proportionally. Serum creatinine rises — because there's more of its precursor, not because kidneys are failing to clear it.

The kidneys are working normally. The marker moved for a different reason than usual. That's the entire mechanism behind the claim.

What the safety research shows

Creatine's safety has been examined about as thoroughly as any supplement.

The International Society of Sports Nutrition's position stand concludes that creatine supplementation does not cause kidney damage or renal dysfunction in healthy individuals. Studies monitoring kidney function markers over months to years — including trials running up to five years — have not found deterioration attributable to creatine.

Work specifically examining glomerular filtration rate using more direct measures than serum creatinine has generally found no impairment.

There are case reports of kidney problems in people taking creatine. Case reports are hypothesis-generating rather than causal evidence, and they typically involve pre-existing conditions, other supplements, or dehydration. Controlled trials in healthy people consistently haven't reproduced a problem.

The practical implication: tell your doctor

This is the genuinely useful action item from this whole post.

If you have blood work done while supplementing creatine, your serum creatinine may read elevated, and your estimated GFR — which is calculated from serum creatinine — may read lower. A physician who doesn't know you supplement may reasonably interpret that as reduced kidney function and order further investigation.

Two things help:

Mention it before testing. "I take 3 grams of creatine daily" is all that's needed. Many clinicians know this interaction; many haven't encountered it.

Ask about cystatin C if there's genuine concern. Cystatin C is an alternative kidney function marker that isn't affected by muscle mass or creatine intake, and it can distinguish a supplementation artefact from actual impairment.

Who should be cautious

Being specific, because "it's safe" without qualification isn't honest.

  • Existing kidney disease or reduced kidney function. Safety data covers healthy adults. This is a nephrologist's call, not a blog's.
  • Single functioning kidney, or a history of kidney problems.
  • Medications affecting kidney function — including NSAIDs at high or sustained doses, some diuretics, and certain other drugs. Ask a pharmacist; they're accessible and this is exactly what they're for.
  • Diabetes with kidney involvement.

Two related concerns, briefly

Dehydration and cramping. The theory was that creatine draws water into muscle and leaves less available elsewhere. Controlled trials — including in athletes exercising in heat — have generally found no increase in cramping, dehydration or heat illness, and some found the opposite. See creatine and hydration.

Liver damage. Long-term monitoring studies tracking liver enzymes alongside kidney markers haven't found evidence of hepatic injury in healthy individuals.

The honest summary

In healthy adults, at standard doses, creatine has an unusually strong safety record — better documented than most things people take without a second thought.

It does raise a common lab marker for a benign reason, and that's worth telling your doctor. And "safe in healthy adults" is not "safe for everyone", which is why the list above matters.
