Does Creatine Cause Hair Loss? Tracing the Claim
One study of twenty rugby players, seventeen years ago, measured a hormone — not hair. Here's what happened next.

The creatine–hair loss claim is unusually traceable. It comes from a single study, it measured something other than hair, and it has been repeatedly examined since.
Worth walking through, because it's a good case study in how a plausible mechanism becomes an internet certainty.
The 2009 study
A South African trial involving college-aged rugby players. Participants took a creatine loading protocol followed by maintenance, and researchers measured hormone levels.
The finding: dihydrotestosterone (DHT) increased — reported as roughly 56% after loading and remaining around 40% above baseline during maintenance. Total testosterone did not change significantly.
That's the entire empirical basis for the claim.
Why DHT is the link
The mechanism is real, which is what gives the claim legs.
DHT is a potent androgen converted from testosterone by the enzyme 5-alpha-reductase. In people genetically susceptible to androgenetic alopecia — male- and female-pattern hair loss — DHT binds to receptors in hair follicles and drives progressive miniaturisation, producing thinner, shorter hairs and eventually none.
This isn't controversial. It's why finasteride, a 5-alpha-reductase inhibitor, works as a hair loss treatment.
So the reasoning runs: creatine raised DHT, DHT causes pattern hair loss, therefore creatine causes hair loss.
The gaps in that chain
Four, and they compound.
1. The study didn't measure hair. Not hair count, not density, not shedding, not follicle miniaturisation. It measured a hormone. Nobody in that trial was assessed for hair loss.
2. It's a small single study. Twenty participants, one population, one protocol. That's hypothesis-generating, not conclusive.
3. Replication attempts have not reproduced it. This is the crucial point. Subsequent studies measuring androgen responses to creatine have generally not found the DHT increase. Systematic reviews examining creatine and androgen levels have concluded the evidence does not support a consistent effect on testosterone or DHT, and reviews looking specifically at hair loss have found no evidence that creatine causes it.
4. Whether the DHT levels stayed within normal range matters. A percentage increase from a low baseline can remain entirely within the normal physiological range. A 56% rise sounds alarming and tells you little without absolute values and reference ranges.
What a fair reading looks like
Not "definitively disproven" — that would be overclaiming in the other direction.
The honest position: one small study found a hormonal change that has not been replicated, and no study has demonstrated that creatine causes hair loss. There is no direct evidence linking creatine supplementation to hair loss outcomes in humans.
Could a subtle effect exist in genetically susceptible individuals that studies haven't been powered to detect? In principle, yes — that's true of many things. But that's a statement about the limits of evidence, not a finding.
The confounder worth naming
Something rarely mentioned in this discussion.
Androgenetic alopecia is extremely common and typically begins in the late teens through thirties in men. That happens to be exactly the demographic most likely to start taking creatine.
So a substantial number of people will begin creatine and notice hair thinning within a year or two — because that's when it was going to start anyway. Temporal coincidence in a highly prevalent condition generates a very large number of personal anecdotes, and personal anecdotes are what sustain this claim online far more than the 2009 paper does.
If you're genuinely worried
Reasonable options, without pretending the uncertainty is zero:
- Assess your own risk. Androgenetic alopecia is strongly heritable. Family history on both sides is more informative than any supplement decision.
- See a dermatologist if you're noticing changes. Early-stage pattern hair loss responds far better to treatment than late-stage. This is worth doing regardless of creatine.
- Decide what you'd trade. If hair matters more to you than a few extra reps, not taking creatine is a completely rational choice. It's a supplement with modest benefits, not a necessity.
That last point is worth stating plainly. We sell creatine, and we'd still rather you make a decision you're comfortable with than one we talked you into.
